A student add/drop form simplifies the initial intake of course change requests.  Most commonly used higher education, this form may be used as the starting point form more intricate multi-step approvals, or may be used as is.  Demo the form to get an email copy of the completed request.

A basic expense report form provides data entry for multiple expense line items and summarizes amounts including Report Total, Total Due to Employee, and Total Billable to a Customer.  The expense report also include an adjustable mileage calculation based on IRS reimbursement rates.

The Federal W-4 demonstrates a multi step form with calculations working across each step, and the use of external styles to present the web form in an identical representation of the government PDF version of the form.  The demo can return an email copy of the completed form if an email address is provided.

The 2020 version of the Federal W-4 contains many differences from past years, resulting in a completely new version of the form.  The 2020 version still demonstrates the use of externals CSS and image references, in addition to offering a preview function, giving the user the ability to visually review the completed, pixel perfect rendition of the document.  This form and its workflow are available for download and use by any GlobalForms customer in the Square 9 SDN for GlobalCapture.

The patient screening form demonstrates a guided form process that takes the user through a series of standard questions leading up to an MRI.  Multiple pages are used to guide the user through the necessary steps, including the ability to mark up the location of metal objects in the body use mouse, stylus, or finger.  Be sure to provide an email address to receive a copy of the complete form.

Using a simple form process, users can easily scan or attach a document and convert it to a variety of editable or archive document formats.  This example allows the user to choose what type of document to create from their Tiff, PDF, or JPG attachment.

This multi-part form demonstrates using data entered by the user in subsequent form steps.  The first name, last name, and address elements from the first step are using to construct user specific language in the second step.  Additionally, this form demonstrates data going straight to email, without the need to create / map an additional document.  

A single page form example that demonstrates W-9 intake.  While the W-9 form serves both business (Tax ID) and individual (SSN) use cases, the example removes ambiguity by ensuring only the correct selections can be made for the given use case.  Like the W-4, this form demonstrates the use of CSS to style and position form elements to represent the paper form as closely as possible.

This simple form can be integrated into a company's vendor on-boarding process.  Requests for taxpayer identification can be manually or automatically emailed to a vendor, who can then access the form and attach a W-9.  The W-9 is securely archived, and accounting can continue the payment process.
